Croydon Flow Team

How our service can help you

The Croydon Flow team works within the borough of Croydon to manage and support clinical decisions around service users movements through the healthcare service.

We work closely with other services both inside and outside of South London and Maudsley NHS Foundation Trust to ensure the best care outcome is achieved for every service user.

We work closely with the South London And Maudsley (SLAM) bed management structure and across teams to effectively manage and support services users' safe discharge from our in-patient settings in a timely manner.

The team provides Home Treatment Assessment, suitable follow up no later than 48 hours after discharge from in-patient wards and support for in-patient and community teams which encourages; right care, at the right time, at the right place, considering the most least restrictive options at all times.

The team consists of; Advanced Nurse Practitioners, Senior discharge coordinators Home treatment assessors, HAWK advisors, and a Community Clinical Service Lead who works proactively with care coordinators and community team leaders for swift support.
