Summary:
The Homeless service (Kings Health Partners) works with homeless people admitted to hospital. The SLaM team is part of a wider team that works across all the KHP hospitals. The team aims to improve both the quality of both the in-patient stay, and discharge arrangements for homeless people.

The team provides expert review for homeless people with mental health conditions who also have coexisting physical health and/or addictions presentations – particularly where there is a history of these needs not being met in the community. The team also provides advice and advocacy around housing law (especially at local authority homeless persons units), and will work to reconnect clients, nationally and internationally (including clients with no recourse to public funds).

Referral criteria:
• 18+
• Patients living in homeless hostels, B&B, sofa surfers or who have nowhere to go on discharge
• Patients with any form of mental illness
• Patients currently admitted to hospital or who are known frequent attenders to crisis services

In general the team would not expect to work directly with clients who already have a care coordinator, but will be happy to give advice where this required.
